- Legalized extract from the trade register of the country of origin or other equivalent proof of legal status of the organization in accordance with the laws of the country of its establishment (the extract must be dated no later than one year prior to applying for state registration), with translation into Belarusian or Russian (the translator's signature must be notarized ) - for the founders who are foreign organizations;

- Copy of identity document, with translation into Belarusian or Russian (the translator's signature must be notarized) - for the founders are foreign individuals;
- The original or a copy of payment document confirming payment of state fees.

LIST OF DOCUMENTS PROVIDED TO THE STATE registration of amendments made to the CHARTER LIMITED LIABILITY COMPANY

For state registration of changes and amendments to the charter of a limited liability company, the registering body:
- The application for state registration;
- Changes and (or) add in two copies, which must be added as annexes to the ordinance, without notarization, their electronic copy (in formate.doc ili.rtf). At the request of the charter of the organization can be represented in the new edition;
- The original certificate of state registration in case of change of name of the organization or reorganization;
- The original or a copy of payment document confirming payment of state fees.

- Legalized extract from the trade register of the country of origin or other equivalent proof of legal status of the organization in accordance with the laws of the country of its establishment (the extract must be dated no later than one year prior to applying for state registration), with translation into Belarusian or Russian (the translator's signature must be notarized ) - for the parent who is a foreign organization;
- Copy of identity document, with translation into Belarusian or Russian (the translator's signature must be notarized) - for the parent who is a foreign physical person;
- The original or a copy of payment document confirming payment of state fees.

Application for state registration shall be signed by:
at the state registration of legal entity created by the establishment - the property owner or its founders (participants) of the entity. If the number of founders (participants) of the commercial, nonprofit organization for more than three, they have the right to authorize one of them to sign declarations of what must be stated in the document evidencing the intention of establishing a commercial organization.
at the state registration of an individual entrepreneur - a citizen registered as a sole proprietor;
for state registration of alterations and (or) amendments to the articles of association (memorandum - for a commercial organization serving only on the basis of the Memorandum of Association) - the legal entity or other person authorized in accordance with the statute (the memorandum), or power of attorney to act on behalf of the entity;
for state registration of alterations and (or) amendments to the certificate of state registration of an individual entrepreneur - an individual entrepreneur.
If the application is submitted by such persons in person, the authenticity of the signatures on the application for state registration shall be certified by an authorized employee of the registering authority that carried out the acceptance of documents. In other cases, the signatures of these individuals, on the application for state registration must be certified by a notary.
When applying for state registration of citizens to submit documents verifying identity and proof of their credentials if they represent the interests of the entity or person.

LIST OF DOCUMENTS SUBMITTED FOR IDENTIFICATION OF CUSTOMS NUMBERS (for legal entities)

1. Statement for the identification of customs facilities.
2. A copy of the notification on a payer account number (UNP).
3. Copy of Certificate of Registration.
4. A copy of the charter company (LLC, SLC, etc.), in the absence of a statute of complete data on the founders - a copy of the memorandum of association.
5. Certificate from Bank / Bank of the existence of accounts (including bank MFO), certified by a round stamp of the bank with the list of persons entitled to sign payment documents.
6. Copies of passports of the head and chief accountant (the page with passport number, date issued, pages 30 - 31, 32, residence).

Copies of documents must necessarily be certified by a round stamp participant in foreign economic activities. Copies of the documents on thermal paper (taken on the fax machine) will not be accepted.

LIST OF DOCUMENTS SUBMITTED FOR IDENTIFICATION OF CUSTOMS number (for individual entrepreneurs)

1. Statement for the identification of customs facilities.
2. A copy of the notification on a payer account number (UNP).
3. Copy of Certificate of Registration.
4. Certificate from Bank / Bank of the existence of accounts (including bank MFO), certified by a round stamp of the bank with the list of persons entitled to sign payment documents.
5. A copy of the passport owner (the page with passport number, date issued, pages 30 - 31, 32, residence).

Copies of documents must necessarily be certified by a round stamp participant in foreign economic activities. Copies of the documents on thermal paper (taken on the fax machine) will not be accepted.

List of Documents provided by legal entities to the Tax authorities for registration of legal person as a TAXPAYER

To formulate the legal entities registered with tax authorities (except for legal persons, information about state registration of which was obtained from the registration authorities), and issuing the notification on a registration number of the payer to the tax authority where the legal entity shall be submitted:
- The application;
- A copy of the constituent documents with a stamp of authority, the state registration of legal entity;
- State registration certificate;
- Copies of documents confirming the powers of the head entity, person performing his duties, notarized, or by presenting the original tax authority for verification;
- Copies of documents confirming the authority of the person directing accounting entity - the chief accountant, bookkeeper (in the absence of a state chief accountant), manager (accountant, chief accountant), an organization that provides services for accounting and reporting, or specialist accountant, is an individual entrepreneur (the order of appointment, an excerpt from an employment agreement (contract) on the term of the contract, contract for services), notarized, or by presenting the original tax authority for verification;

Documents are presented in the original or a notarized copy, and return to a legal entity, individual entrepreneur.

Organizations of the Republican Unitary Telecommunication Enterprise Beltelecom, individual entrepreneurs - payers of the single tax from individual entrepreneurs and other individuals and organizations of the Belarusian Railways, using interstate automated control system "Express", for the registration of devices and systems book cashier-teller does not appear.
